Recently published consensus recommendations on pediatric decision-making by Salter and colleagues (2023) did not address neonatal decision-making, due to the unique complexities of neonatal care. This essay explores three areas that impact neonatal decision-making: legal and policy considerations, rapid technological advancement, and the unique emotional burdens faced by parents and clinicians during the medical care of neonates. The authors evaluate the six consensus recommendations related to these considerations and conclude that the consensus recommendations apply to neonates.

Many theories of complex cognitive-motor skill learning are built on the notion that basic cognitive processes group actions into easy-to-perform sequences. The present work examines predictions derived from laboratory-based studies of motor chunking and motor preparation using data collected from the real-time strategy video game StarCraft 2. We examined 996,163 action sequences in the telemetry data of 3,317 players across seven levels of skill. As predicted, the latency to the first action is delayed relative to the other actions in the (...) group. Other predictions, inspired by the memory drum theory of Henry and Rogers, received only weak support. (shrink)

We consider the varieties and directions of influence that the source and target domains involved in a conceptual metaphor can have on each other during the course of understanding metaphorical utterances based on the metaphor. Previous studies have been restricted both as to direction of influence and as to type of influence. They have been largely confined to the “forward” (source to target) direction of influence, and they have concentrated on the transfer of features or propositions and (to some extent) (...) the highlighting of aspects of a domain. By contrast, this article stresses the importance both of other varieties of influence (e.g., transfer of queries and uncertainty effects) and of “reverse” influence (target to source). We seek to curb the natural tendency to think that, because metaphor involves an overall move from source to target, therefore all inter-domain influences in the course of understanding go in that direction. The bulk of the article explores the theoretical issues involved. These issues have arisen out of a reasoning-based approach to metaphor, seeking to make best use of information from metaphorical utterance, context, source and target domains, and known metaphorical mappings, by applying a complex overall reasoning process. The article briefly explains the thorough implementation of reverse influence in a computer program for metaphorical reasoning called ATT-Meta, although the theoretical considerations are relevant to all disciplines concerned with metaphor. Watching another person in pain activates brain areas involved in the sensation of our own pain. Importantly, this neural mirroring is not constant; rather, it is modulated by our beliefs about their intentions, circumstances, and group allegiances. We investigated if the neural empathic response is modulated by minimally-differentiating information (e.g., a simple text label indicating another’s religious belief), and if neural activity changes predict ingroups and outgroups across independent paradigms. We found that the empathic response was larger when participants viewed (...) a painful event occurring to a hand labeled with their own religion (ingroup) than to a hand labeled with a different religion (outgroup). Counterintuitively, the magnitude of this bias correlated positively with the magnitude of participants’ self-reported empathy. A multivariate classifier, using mean activity in empathy-related brain regions as features, discriminated ingroup from outgroup with 72% accuracy; the classifier’s confidence correlated with belief certainty. This classifier generalized successfully to validation experiments in which the ingroup condition was based on an arbitrary group assignment. Empathy networks thus allow for the classification of long-held, newly-modified and arbitrarily-formed ingroups and outgroups. This is the first report of a single machine learning model on neural activation that generalizes to multiple representations of ingroup and outgroup. The current findings may prove useful as an objective diagnostic tool to measure the magnitude of one’s group affiliations, and the effectiveness of interventions to reduce ingroup biases. (shrink)

And yet alongside these insights there is a theory of the nature of the relation between language and reality which appears both to be impossible to work out in detail in a (...) way which is completely satisfactory, and to be bizarre and incredible. I am referring to the so-called logical atomism of the Tractatus . The main outlines of this theory at least are clear and familiar: there are elementary propositions which gain their sense from being models of possible states of affairs; such propositions are configurations of names of simple objects, signifying that those simples are analogously configured; every proposition has its sense through being analysable as a truth-functional compound of elementary propositions, thus deriving its sense from the sense of the elementary propositions when this view is taken in conjunction with the idea that the sense of a proposition is completely specified by specifying its truth-conditions. In this way the Tractatus incorporates in its working out a philosophical system analogous to the classical philosophical systems of Leibniz or Spinoza which are regarded by many people, in a sense rightly, as the prehistoric monsters of philosophy which are not to be studied as living organisms, but studied as the curiosities of human thought. And we may here agree that in the end we must simply reject a philosophy which incorporates such features as its postulation of simple eternal objects, or of a possibility of an analysis of a proposition which was presented as a pre-condition for the propositions that we ordinarily utter to make sense, and yet the specific form of which we are unaware of, and so on. An analysis of Mark Bevir's account of the role of language and tradition on the one hand, and the individual on the other in the generation of ideas, and proposal of an alternative account It endorses Bevir's project of finding a middle way between individualism and collectivism, but finds that Bevir exaggerates the role of the individual. It argues that human selves always remains dependent on language even to articulate their own intentions to themselves. Whilst they have a capacity (...) to create new linguistic expressions, this is always limited to the exploration of possibilities already latent in the language. However, no one is a mere recipient and conduit of a given language: everyone hands it on transformed by their unique appropriation of it. The antifoundationalist analyses of Wittgenstein, Newman Collingwood, and Neurath are invoked to argue that this state of affairs also applies to the individual's relation to the beliefs and values inherited traditionally: there is no possibility of a wholesale rejection of what is received; no individual can reject all received traditions, and erect an entire belief structure from scratch, but can only modify it on a piecemeal basis, so that received tradition always remains constitutive of the individual mind. It is also argued that human self-consciousness is always socially formed, and no person ever completely integrated, and stabilized. No one is ever therefore in a state of complete self-possession. One therefore must reject Bevir's claim that the historian of ideas must initially presume that individuals are sincere, conscious and rational in their expressed beliefs: ‘sincere’ self-consciousness is an ideal never fully achieved, and beliefs as to what constitutes ‘rationality’ are so varied that specific presumptions cannot be made. This is a fresh and stimulating analysis of the esthetic experience in terms of the import it gives to the role of "affective hazard" in the constitution of the esthetic form. The author, who comes with a background in English literature, proposes that all esthetic experiences have one common feature, their form as felt unity which endows the object with a value that distinguishes it from other objects. The experiencer as one of the terms of the relationship is confronted with (...) a manifold of elements one of which stands out in a hazardous way as an impediment to the union of the terms. Its very hazardousness, however, impregnates the whole structure, and in spite of an apparent obstruction to the order it actually bestows a balance and symmetry that would otherwise be lacking. There is a bending, but no breaking; the fragility itself is a necessary precondition for the form that constitutes the esthetic experience marking the object as a potentiality for some form yet unactualized but implicitly determined. The scholastic notion of being is evident in Slattery’s analysis of form as the principle of "unity," "intelligibility," and "determination." Rather than emphasize the static quality of things she chooses to focus on the object as both open and closed; open to receive many forms but closed in that it is only actualized as one form by a mind who at will chooses it from a manifold of other possible forms which it rejects. Unlike the Aristotelian form which inheres in the thing independent of the experiencer the form in the esthetic object is constituted by an experiencer who is an active participant in the relationship. This active term also confers upon the object its value which defines it as an object of esthetic quality. Interspersed throughout the book are descriptive analyses of various esthetic forms that illustrate what Slattery means by the decisive role of "affective hazard," and it is here that she is at her best. So well does she capture the essence of the dynamic richness peculiar to the esthetic object that she evokes in the reader an affective excitement and an awareness of a felt unity. This in itself verifies her thesis, and we could only have wished that she furnished us with more of the heightening experiences.—K. R. M. (shrink)

If there is an age in which philosophy seemed to experience a demise it is the nineteenth century, and yet this was not due to a lack of philosophy nor to the fact that there prevailed an attitude of estrangement from philosophy. Rather, what appeared to be a de-emphasis was merely a replacement of writings by "philosophers" with those by the natural scientist and the humanist. Tatarkiewicz divides his period into three phases distinguishing the era with their peculiar disciplines: 1830-60 (...) marked by positivism, empiricism, naturalism, and dialectical materialism; 1860-80 in which the minimalistic philosophy of positivism reached full bloom; 1880-1900 dominated by minimalism, positivism, scientism and in addition the appearance of opposing doctrines. Although such familiar names as Comte, Mill, Marx, Engels, Spencer, Brentano and Nietzsche find their way into Tatarkiewicz’ book, of greater significance is the manifold of less familiar figures who play a determining role in this period of philosophy. Yet it is not the author’s intent to expound individual doctrines in reference to anything but philosophical currents of the day. Tatarkiewicz focuses on times not men. A very good summary is presented at the end of the book mapping out the social, economic, political, geographic, and religious climate prevalent in the nineteenth century, thus concluding a helpful survey of a period that has been somewhat neglected in the history of philosophy.—K.R.M. Philosophers committed to the task of coming to grips with reality must face the fact that there are no final solutions and the need to question is fundamental to their project. Taking this as his point of departure Clark proposes that questioning is not confined to the philosopher; it marks every self that is confronted with a given empirical order. Before rendering an analysis of the experience of questioning which is the main thrust of this work, Clark outlines the situation (...) of knowledge in which we find two prevailing viewpoints, the spectator approach, and the performative approach. In the former a subjectivism ensues when the knower passively receives the objects creating a solipsistic prison where the objects take residence precluding any response to the world. In this sense there is no appropriation of meanings as "mine." Clark maintains that this is not what occurs. The self does not merely stand against a world of objects, rather through his intentions he modifies the data conferring meaning on it and in doing so validates his own self. The performative account acknowledges the knower as an active constituting agent who not only intends the facts but uses them in his effort to become a self. A series of historical studies is presented examining these two viewpoints in such philosophers as Plato, Aristotle, Aquinas, Descartes, the empiricists, and finally ending with Kant who represents the synthesis of both perspectives. Using the Critiques as his source, Clark suggests that while the theoretical reason of Kant is a spectator approach, practical reason incorporates the performative account where the "I can" verifies the active self who is responsible for its realization. Kant’s endeavor to construct a transcendental analysis is seen by Clark as primarily an enterprise to set up the necessary conditions for knowledge which can be applied in the practical order. The self only questions for a reason, so that it "can" do something. The "I can" presupposes the "I know," but the "I know" presupposes the "I ask." Clark proposes his own transcendental analysis of questioning and asks what structures are necessary for the experience of questioning in general with total disregard for the content of that question. He attempts to show that the experience involves self discovery, for what is revealed will only be revealed through further self questioning which is always subject to modification. Questioning is a unique and personal response to a world in which I find myself and yet I intend the world, bestowing on it the meaning that is necessary for my fulfillment. There is an analogy between this transcendental analysis and Kant’s, although Clark is quick to point out that he has hesitations about Kant’s treatment of time and space which he has somewhat modified to serve as the necessary structures of the experience of questioning. Philosophy has been pregnant with analyses of the self primarily as a knowing agent and perhaps it is now time for a movement towards the direction that Clark is suggesting. Historically, entities with a vested interest in a product that critics have suggested is harmful have consistently used research to back their claims that the product is safe. Prominent examples are: tobacco, lead, bisphenol A, and atrazine. Research literature indicates that about 80–90% of studies with industry affiliation found no harm from the product, while only about 10–20% of studies without industry affiliation found no harm. In parallel to other historical debates, recent studies examining a possible relationship between mercury exposure (...) and autism spectrum disorder show a similar dichotomy. Studies sponsored and supported by industry or entities with an apparent conflict of interest have most often shown no evidence of harm or no “consistent” evidence of harm, while studies without such affiliations report positive evidence of a Hg/autism association. The potentially causal relationship between Hg exposure and ASD differs from other toxic products since there is a broad coalition of entities for whom a conflict of interest arises. These include influential governmental public health entities, the pharmaceutical industry, and even the coal burning industry. This review includes a systematic literature search of original studies on the potential relationship between Hg and ASD from 1999 to August 2015, finding that of the studies with public health and/or industry affiliation, 86% reported no relationship between Hg and ASD. However, among studies without public health and/or industry affiliation, only 21% find no relationship between Hg and ASD. The discrepancy in these results suggests a bias indicative of a conflict of interest. (shrink)

The earlier volume in this series dealt with two religions of Indian origin, namely, Buddhism and Jainism. The Indian religious scene, however, is characterized by not only religions which originated in India but also by religions which entered India from outside India and made their home here. Thus religious life in India has been enlivened throughout its history by the presence of religions of foreign origin on its soil almost from the very time they came into existence. This volume covers (...) three such religions—Zoraoastrianism, Judaism, and Islam. In the case of Zoraostianism, even its very beginnings are intertwined with India, as Zoroastrianism reformed a preexisting religion which had strong links to the Vedic heritage of India. This relationship took on a new dimension when a Zoroastrian community, fearing persecution in Persia after its Arab conquest, sought shelter in western India and ultimately went on to produce India’s pioneering nationalist in the figure of Dadabhai Naoroji ( 1825-1917), also known as the Grand Old Man of India. Jews found refuge in south India after the destruction of the Temple by the Romans in 70 C.E. and have remained a part of the Indian religious scene since then, some even returning to Israel after it was founded in 1948. Islam arrived in Kerala as soon as it was founded and one of the earliest mosques in the history of Islam is found in India. Islam differs from the previously mentioned religions inasmuch as it went on to gain political hegemony over parts of the country for considerable periods of time, which meant that its impact on the religious life of the subcontinent has been greater compared to the other religions. It has also meant that Islam has existed in a religiously plural environment in India for a longer period than elsewhere in the world so that not only has Islam left a mark on India, India has also left its mark on it. Indeed all the three religions covered in this volume share this dual feature, that they have profoundly influenced Indian religious life and have also in turn been profoundly influenced by their presence in India. Factors such as subsistence turnover, warfare, or interaction between different groups can be major sources of cultural change in human populations. Global demographic shifts such as the transition to agriculture during the Neolithic and more recently the urbanization and globalization of the twentieth century have been major catalysts for cultural change. Here, we test whether cultural traits such as patri/matrilocality and postmarital migration persist in the face of social upheaval and gene flow during the past 150 years in postcolonial South (...) Africa. The recent history of South Africa has seen major demographic shifts that resulted in the displacement and forced sedentism of indigenous Khoekhoe and San populations. During the expansion of the colonial frontier, the Khoe-San admixed with European colonists and enslaved individuals from West/Central Africa, Indonesia, and South Asia, introducing novel cultural norms. We conducted demographic interviews among Nama and Cederberg communities representing nearly 3,000 individuals across three generations. Despite the history of colonial expansion, and the subsequent incorporation of Khoe-San and Khoe-San-descendant communities into a colonial society with strong patrilocal norms, patrilocality is the least common postmarital residence pattern in our study populations today. Our results suggest that more recent forces of integration into the market economy are likely the primary drivers of change in the cultural traits examined in our study. Birthplace had a strong effect on an individual’s odds of migration, distance moved, and postmarital residence form. These effects are at least partially explained by the population size of the birthplace. Our results suggest that market factors local to birthplaces are important drivers of residence decisions, although the frequency of matrilocal residence and a geographic and temporal cline in migration and residence patterns also indicate the persistence of some historic Khoe-San cultural traits in contemporary groups. If stem cell-based therapies are developed, we will likely confront a difficult problem of justice: for biological reasons alone, the new therapies might benefit only a limited range of patients. In fact, they might benefit primarily white Americans, thereby exacerbating long-standing differences in health and health care.

The Adverse Outcome Pathway (AOP) concept is a knowledge assembly and communication tool to facilitate the transparent translation of mechanistic information into outcomes meaningful to the regulatory assessment of chemicals. The AOP framework and associated knowledgebases (KBs) have received significant attention and use in the regulatory toxicology community. However, it is increasingly apparent that the potential stakeholder community for the AOP concept and AOP KBs is broader than scientists and regulators directly involved in chemical safety assessment. In this paper we (...) identify and describe those stakeholders who currently—or in the future—could benefit from the application of the AOP framework and knowledge to specific problems. We also summarize the challenges faced in implementing pathway-based approaches such as the AOP framework in biological sciences, and provide a series of recommendations to meet critical needs to ensure further progression of the framework as a useful, sustainable and dependable tool supporting assessments of both human health and the environment. Although the AOP concept has the potential to significantly impact the organization and interpretation of biological information in a variety of disciplines/applications, this promise can only be fully realized through the active engagement of, and input from multiple stakeholders, requiring multi-pronged substantive long-term planning and strategies. Ethical decision-making in public health rarely involves simply avoiding a bad choice in favor of a good choice. Instead, it requires policymakers to strike a balance among conflicting goals that are all good—goals such as the health of populations and individuals, knowledge gained through scientific research, autonomy, social justice, and the efficient use of limited resources. This balance can be elusive, and perfect examples are the legal instruments governing dual-use research, a term describing scientific endeavors meant to produce beneficial knowledge (...) or technology, but that could also be misapplied. This case is another in a series intended to highlight the new questions emerging from advances in mapping the human genome and the application of genetic findings to clinical practice. The National Human Genome Research Institute, a component of the National Institutes of Health, by law is directed to designate a portion of its annual budget to furthering understanding of the ethical, legal, and social questions emerging from research on the human genome. As part of the effort, the Institute supports (...) research by scientists and scholars around the nation with the aim of clarifying and resolving the tough ethical and research choices facing this endeavor. But recently it has launched an intramural program, which is expected to take a catalytic role in grappling with the array of issues the researchers face in carrying out investigations in human genetics. Extending previous research on the characteristics associated with adolescents’ general tendency to be a moral rebel, the present study examined the roles of moral identity and moral courage characteristics on 3 expressions of the tendency to stand up for one’s beliefs and values despite social pressure not to do so. Results revealed that general and situation-specific moral courage characteristics are important motivators of individuals’ caring, just, and brave expressions of the tendency to be a moral rebel, especially when they possess (...) a relatively strong moral identity. Objective: To determine whether the marks in the third year Objective Structured Clinical Examination were affected by the collusion reported by the students themselves on an electronic discussion board.Design: A review of the student discussion, examiners’ feedback and a comparison of the marks obtained on the 2 days of the OSCE.Participants: 255 third year medical students.Setting: An OSCE consisting of 15 stations, administered on three sites over 2 days at a UK medical school.Results: 40 students contributed to the discussion on (...) the electronic discussion board. The main points raised were perceived inequity between students who did, or did not, have prior knowledge of the station content, and the lack of honesty and professionalism of their peers. Most contributors claimed to have received, or knew of others receiving, prior knowledge, but none confessed to passing on information. No significant difference was observed in the overall mark for the OSCE on day 1 ) and day 2 ). On day 2, marks were considerably greater for four stations and markedly lower for three stations. It was not obvious why collusion should affect these station marks. A clear indication of the effects of collusion could only be obtained from a single subsection of an individual station where 82 students on day 2 incorrectly gave the diagnosis from day 1.Conclusion: Marks do not provide a sound inference of student collusion in an OSCE and may mask the aspects of professional development of students.
